Balanced Living Therapy LLC

Effective November 1, 2026.

Washington's My Health My Data Act gives you rights over health information that businesses collect about you outside of the health care relationship itself. This policy explains what Balanced Living Therapy LLC collects through this website and when you reach out to us, what we do with it, and how to use your rights. It covers balancedlivingtherapy.com and the ways you can contact us from it.

What this website collects

This site has no analytics, no advertising pixels, no cookies, and no third-party scripts or embedded content. It does not track which pages you read or try to recognize you when you come back. The only thing that happens when you load a page is the ordinary request your browser sends to our hosting provider for that page, which includes your device's network address; our hosting provider uses it to deliver the page and keeps it only as part of normal, short-lived server logs. We do not use that information to identify you, and we do not sell or share it.

One page collects information: our intake form at balancedlivingtherapy.com/intake. What you enter there, including any photos of your insurance card, goes straight from your browser to the practice's own Microsoft systems, which are covered by a business associate agreement. Our website hosting provider never receives it. To stop automated abuse, the form briefly uses a scrambled form of your device's network address to limit how many times it can be sent; that is deleted within a day and is never stored with your answers. The form also counts how many people start it and how many finish it. Those counts contain no names, contact details, answers or network addresses. If you reached the form from one of our emails, the link carries a random code that lets us attach your answers to your inquiry; the code contains no information about you, and the form never shows you anything we already have on file.

What you send us when you reach out

If you fill in our intake form, email us, or call and leave a message, you choose what to tell us. That usually includes:

  • Your name and how to reach you
  • Who the care is for and what you're looking for help with
  • Your insurance, if you want us to check your coverage, and photos of your insurance card

We use it for one purpose: to respond to you, check your coverage, and match you with a clinician. The source is you. It is held inside the practice's Microsoft 365 and Microsoft Azure accounts, under a business associate agreement, and is seen by our front desk and the clinicians involved in your care. We do not sell it, and we do not share it with anyone outside the practice except the companies that run our records and messaging systems under those agreements, or as the law requires.

Your health information as a client

Once you are a client, the information we hold about your care is protected health information under HIPAA, and substance use treatment records have the added protections of 42 CFR Part 2. The My Health My Data Act does not cover that information; our Notice of Privacy Practices does, and it explains your rights over your records in detail.

Text messages

If you sign up for texts from us, on our intake form or by asking us, we use your mobile number only to text you about scheduling, appointments, and paperwork. We do not sell or share your mobile number or your texting consent with anyone for marketing, and we will not share your opt-in to text messages with any third party for purposes unrelated to sending you those texts. Every category of sharing described in this policy excludes text messaging originator opt-in data and consent; that information is not shared with any third party. Our texting provider handles messages for us under a business associate agreement. You can stop texts at any time by replying STOP. The full texting terms are on our texting page.

Your rights

For any consumer health data this policy covers, you have the right to:

  • Confirm whether we are collecting, sharing, or selling consumer health data about you, and to see it
  • Withdraw consent for us to collect or share it
  • Have it deleted
  • Not be discriminated against for using these rights

To use any of these rights, contact our Privacy Officer, Jesse Dunn, at (425) 977-4988 or frontdesk@balancedlivingtherapy.com, or write to Balanced Living Therapy LLC, 7500 212th Street SW, Suite 205, Edmonds, WA 98026. We will answer within 45 days, and we will tell you if we need more time. If we deny a request, we will say why and how to appeal to us. If you are not satisfied with the outcome of an appeal, you can contact the Washington State Attorney General's Office.

Changes to this policy

If this policy changes, the new version will be posted here with a new effective date.
